Job summary

Monash Health in Berwick, VIC, is seeking a Grade 2 Social Worker for a full-time fixed-term role at Casey Hospital until August 2027. You will deliver high-quality psychosocial assessments and support patients and families during challenging times.

Work across general medicine, surgery, Emergency, Women’s and Children’s, with multidisciplinary teams to ensure safe, person‑centred discharge planning and community connections.

Qualifications

• AASW‑accredited Bachelor or Master of Social Work with eligibility for membership
• Minimum two years post‑graduate Social Work experience
• Strong psychosocial assessment and discharge planning skills
• Excellent communication and time management
• Ability to work autonomously within a multidisciplinary team
• Cultural and linguistic diversity awareness

Responsibilities

• Undertake comprehensive psychosocial and risk assessments
• Support complex discharge planning and care coordination
• Provide counselling, advocacy and practical support to patients and families
• Work with multidisciplinary teams to support safe, person‑centred care
• Connect patients with community services and supports
• Contribute to ongoing development of Social Work practice
