Care Manager role plays an important role in the leadership team and is committed to creating opportunities for our clients, team and the community.
We are seeking a compassionate, experienced, professional Care Manager to support a female NDIS participant who requires assistance with personal care. This role is open to female applicants only to ensure our client's comfort, dignity and psychological safety.
Key Responsibilities
- Participant Relationship Management: Build and maintain meaningful relationships with participants, ensuring their needs and goals are met through regular communication and personalized care approaches.
- Care Plan Development: Collaborate with participants, their families, and healthcare professionals to design and implement tailored support plans that align with NDIS requirements.
- Support Worker Oversight: Provide guidance and mentorship to support workers, fostering a culture of continuous improvement and excellence in service delivery.
- Client Communication: Act as the primary point of contact for participants and their families, addressing inquiries and resolving concerns with sensitivity and efficiency.
- Staff Training and Development: Identify training needs and lead initiatives to enhance the skills and knowledge of support workers, ensuring high standards of care.
- Client Onboarding: Oversee the seamless onboarding process for new participants, including initial assessments and orientation to our services.
- Compliance and Documentation: Maintain meticulous and up-to-date client records, ensuring compliance with all regulatory and organizational policies. Report writing and incident management.
- Stakeholder Engagement: Collaborate with external stakeholders, including healthcare providers, therapists, and funding bodies, to deliver holistic support.
- Operational Support: Participate in a rotating on-call schedule to ensure the continuity of high-quality care for our participants outside of regular business hours.
Requirements
We are seeking a compassionate and driven individual with the following qualifications and attributes:
Essential Qualifications
- A relevant tertiary qualification in disability, social work, health, or a related field.
- Current NDIS Worker Screening Check and a valid Blue Card (or ability obtain).
- Current driver's license and reliable vehicle.
Strongly Desirable
- Experience with complex care, including co-morbidity, mental health and chronic health concerns advantageous.
- Qualifications in nursing and/or mental health (or willing to gain).
Professional Experience
- Proven track record in care management or a similar role within the disability or health sector.
- Strong understanding of NDIS practices, regulations, and guidelines.
- Experience in supervising and mentoring staff.
Skills and Competencies
- Outstanding interpersonal and communication skills.
- Exceptional organizational skills and attention to detail.
- Ability to manage competing priorities in a fast-paced environment.
- Strong skills in technology, Microsoft programs, with proficiency in care management software and general IT systems.
