Telehealth Aftercare Specialist
As a Telehealth Aftercare Specialist, you will play a crucial role in providing exceptional patient care and ensuring a smooth transition for patients after their visits with healthcare providers. You will be responsible for a wide range of duties, including ordering prescription refills, arranging durable medical equipment (DME) orders, preparing necessary forms, facilitating referrals to specialists, and scheduling follow-up appointments. Your attention to detail, excellent communication skills, and ability to multitask will be vital in delivering high‑quality aftercare services to our patients.
Responsibilities
- Ensure all necessary aftercare letters are written promptly and accurately for providers to review and sign.
- Coordinate and manage the aftercare process for patients who have had telehealth consultations with healthcare providers.
- Assist patients in ordering prescription refills, ensuring timely delivery and adherence to prescribed treatment plans.
- Arrange for the procurement and delivery of durable medical equipment (DME) as per patient needs and physician recommendations.
- Facilitate referrals to specialists or other healthcare providers based on the recommendations of telehealth providers.
- Collaborate with the telehealth team and external stakeholders to ensure seamless communication and follow‑up care.
- Maintain accurate and up‑to‑date patient records, documenting all interactions and interventions.
- Provide exceptional customer service by addressing patient inquiries and concerns promptly and professionally.
- Stay updated on relevant healthcare guidelines, policies, and procedures to ensure compliance and quality of care.
- Collaborate with the telehealth team to identify opportunities for process improvement and contribute to the overall success of the department.
Qualifications
- Proven experience in healthcare administration, telehealth, or a similar role involving patient care coordination.
- Strong knowledge of medical terminology, prescription medication processes, and healthcare referral protocols.
- Familiarity with durable medical equipment (DME) and the ability to coordinate orders effectively.
- Excellent organizational skills and the ability to multitask in a fast‑paced environment.
- Exceptional attention to detail and accuracy in documentation and record‑keeping.
- Outstanding interpersonal and communication skills, both verbal and written.
- Proficient in using electronic health record (EHR) systems and other relevant healthcare software.
- Ability to work independently and collaboratively within a team.
Job Details
- Job Type: Full‑time, Permanent, Work From Home
- Pay: $5–$7/hr (approx. $962–$6,738 per month)
- Hours: 40 per week, Monday – Friday 9:30 AM – 5:30 PM EST
- Expected Hours: 40 per week
