Job summary

Vytalize Health in the UAE seeks a skilled healthcare operations leader to drive value-based care implementations, including population health management and risk-based contracting across partner practices.

You will collaborate with providers and staff to optimize clinical and administrative workflows, deploy and optimize technology solutions such as EMRs and care management tools, and develop programs to improve quality and outcomes while building trust with administrators and clinicians.

Qualifications

• Bachelor's degree in Healthcare Administration, Public Health, Nursing, or related field.
• 3+ years of experience in practice transformation, care coordination, or population health.
• Direct experience working with physician practices in a value-based care environment.
• Strong understanding of value-based care models and primary care operations.
• Ability to manage multiple practice relationships simultaneously.
• Proficiency in using data and performance metrics to guide interventions.

Responsibilities

• Lead the implementation and deployment of value-based care models including population health management and risk-based contracting
• Collaborate with providers and practice staff to optimize clinical and administrative workflows
• Implement and optimize practice use of technology solutions including EHRs and care management tools
• Develop and implement population health management programs for high-risk patient populations
• Partner with practices to execute strategies to achieve quality performance goals
• Facilitate care coordination among healthcare providers, patients, and families
• Develop performance measurement systems to evaluate VBC interventions
• Build strong, trust-based relationships with practice administrators and providers
