Job summary

Responsibilities

• Conduct initial and ongoing health assessments; monitor vital signs.
• Develop, implement and update individualized care plans with the care team.
• Administer medications and treatments per physician orders; monitor for side effects.
• Educate patients and families about conditions and preventive measures.
• Maintain accurate patient records and ensure data integrity in the chart.
