GluCare Integrated Diabetes Center is seeking a dedicated DHA Registered Nurse to join our clinical team in delivering exceptional patient care. The ideal candidate is compassionate, adaptable, and committed to providing high‑quality nursing services in a modern, fast‑paced environment. Experience in chronic disease management, patient education, and digital tools is a plus.
Key Responsibilities
- Provide direct patient care, including vital sign checks, blood glucose monitoring, and patient observations.
- Educate patients on diabetes management, including lifestyle tips and monitoring instructions, ensuring compliance with care plans.
- Prepare examination rooms and medical equipment, ensuring cleanliness and adherence to infection control standards.
- Assist in the preparation and handling of equipment for procedures, wound care, and specimen collection.
- Communicate effectively with patients, providing empathy and clarity to address their questions and concerns.
- Reinforce health education provided by physicians, encouraging self‑care practices and adherence to treatment plans.
- Accurately record and update patient information, treatment plans, and observations in electronic medical records (EMR).
- Report any changes in patient condition to the supervising nurse or physician promptly.
- Adhere to all DHA regulations, nursing guidelines, and GluCare protocols to ensure the safety and well‑being of patients and staff.
- Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support.
Qualifications
- Valid DHA Assistant Nurse License - A MUST
- Minimum of 1-2 years of experience in a clinical setting, preferably in endocrinology, diabetes care, or chronic disease management.
- Knowledge of diabetes management and patient education in lifestyle‑related chronic conditions is preferred.
- Strong communication and interpersonal skills with a patient‑centric approach.
- Basic proficiency in electronic medical records (EMR) systems is preferred.
