GluCare Integrated Diabetes Center is seeking a dedicated DHA Registered Nurse to join our clinical team in delivering exceptional patient care. The ideal candidate is compassionate, adaptable and committed to providing high-quality nursing services in a modern fast‑paced environment. Experience in chronic disease management, patient education and digital tools is a plus.
Key Responsibilities
Patient Care Support
- Provide direct patient care including vital sign checks, blood glucose monitoring and patient observations.
- Educate patients on diabetes management including lifestyle tips and monitoring instructions, ensuring compliance with care plans.
Clinical Procedures Assistance
- Prepare examination rooms and medical equipment, ensuring cleanliness and adherence to infection control standards.
- Assist in the preparation and handling of equipment for procedures, wound care and specimen collection.
Patient Education and Communication
- Communicate effectively with patients, providing empathy and clarity to address their questions and concerns.
- Reinforce health education provided by physicians, encouraging self‑care practices and adherence to treatment plans.
Documentation and Reporting
- Accurately record and update patient information, treatment plans and observations in electronic medical records (EMR).
- Report any changes in patient condition to the supervising nurse or physician promptly.
Operational and Safety Compliance
- Adhere to all DHA regulations, nursing guidelines and GluCare protocols to ensure the safety and well‑being of patients and staff.
- Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support.
Qualifications
- Valid DHA Assistant Nurse License – A MUST.
- Minimum of 1–2 years of experience in a clinical setting, preferably in endocrinology, diabetes care or chronic disease management.
- Knowledge of diabetes management and patient education in lifestyle‑related chronic conditions is preferred.
- Strong communication and interpersonal skills with a patient‑centric approach.
- Basic proficiency in electronic medical records (EMR) systems is preferred.
