Job summary

Qualifications

• Bachelor’s degree in a health-related field.
• 2–5 years of experience in case management or patient coordination.
• Fluency in Arabic and English (written and spoken).
• Strong interpersonal and relationship management skills.
• Ability to coordinate with clients, families, and healthcare professionals.
• Excellent organizational and time management skills.

Responsibilities

• Coordinate and manage client/patient cases for smooth service delivery.
• Act as primary contact for clients, families and healthcare professionals.
• Conduct assessments to identify needs and required services.
• Develop, implement and monitor individualized care plans.
• Coordinate appointments, referrals and follow-ups.
• Maintain accurate case records and documentation.
• Liaise with multidisciplinary teams for continuity of care.
• Provide guidance and information to clients and families.
• Monitor progress and resolve issues promptly.
• Ensure compliance with policies and confidentiality.
• Prepare case summaries and reports for management.
• Support quality improvement initiatives and service excellence.
• Handle inquiries professionally and efficiently.
• Assist in coordinating community resources when needed.
