- Perform comprehensive psychosocial assessments and craft individualised care plans.
- Provide counselling, crisis intervention and ongoing emotional support.
- Facilitate family meetings, discharge planning and community reintegration.
- Advocate for resources, benefits and funding.
- Collaborate with the interdisciplinary team to deliver evidence-based care.
- Maintain accurate documentation compliant with SACSSP standards.

Reporting to the Therapy Manager, our Social Worker is an essential member of the interdisciplinary rehabilitation team.
- Perform comprehensive psychosocial assessments and craft individualised care plans.
- Provide counselling, crisis intervention and ongoing emotional support to patients and families.
- Facilitate family meetings, discharge planning and smooth community reintegration.
- Advocate for resources, benefits and funding to optimise long-term outcomes.
- Collaborate daily with the interdisciplinary to deliver seamless, evidence-based care.
- Maintain accurate, confidential documentation compliant with SACSSP standards and contribute to quality‑improvement initiatives.
- BA Degree in Social Work
- Current SACSSP membership & valid malpractice insurance
- 2-3 years experience in a private hospital or Rehabilitation setting (advantageous)
- Communication - clear, compassionate and culturally sensitive
