A local disability services provider in Gauteng is seeking a Support Coordinator to work with clients with complex care needs. This role involves building relationships with clients and stakeholders while ensuring the implementation of clients' NDIS plans. We provide our services all over Northern and Central Australia, including all rural and remote areas, and have offices located in Darwin, Alice Springs, Broome and Cairns.
The position will be working with clients with complex care and medical needs across the life span, to identify and address barriers to implement their NDIS plan.
The Specialist Support Coordinator is required to build relationships with clients, their family, disability services organisations, clinical staff and various government departments, including health, education, communities and justice. Eunoia Lane encourages a collaborative approach by stakeholders to ensure supports are delivered in a person‑centred and timely.
This position assists with building a foundation for best practice care, staff training, support and intervention for clients, ultimately achieving positive and sustainable outcomes following a period of crisis.
