Placements24 is seeking a dedicated Community Health Worker to serve the Pretoria community. You will educate, support, and navigate individuals to access medical services, acting as a trusted local contact.

The role emphasizes outreach, data collection, liaison with providers, and connecting clients with social services. Candidates should have a high school diploma, CHW certification is a plus, and a valid driver's license.

Qualifications

• High school diploma or equivalent; CHW certification is a plus.
• Experience in community-based health or social services.
• Strong knowledge of local health resources and Pretoria area.
• Excellent communication and interpersonal skills.
• Ability to work independently in the field and build rapport with diverse populations.

Responsibilities

• Conduct health screenings and assessments within the community.

• Collect data on community health needs and service utilization.
