Responsibilities

• Support clients in participating in community activities, social events, educational pursuits, and employment opportunities.
• Assist clients with mobility, transfers, and the use of assistive devices as outlined in their care plans.
• Administer medications and monitor health status, reporting any changes or concerns to supervisors and healthcare professionals.
• Facilitate communication and understanding between clients and their support networks, including family and service providers.
