Key Responsibilities

• Assess client needs and develop tailored support strategies and care plans in consultation with individuals and their families.

• Facilitate clients' engagement in recreational, educational, and vocational activities to enhance their skills and social connections.
• Advocate for clients' rights and needs, ensuring they have access to appropriate services and support networks within the Western Cape.
• Collaborate with healthcare professionals, government agencies, and community organizations to ensure coordinated and comprehensive support.
• Maintain detailed records of client interactions, progress, and any required reporting, adhering to strict confidentiality standards.

Requirements

• Relevant qualification in Social Work, Psychology, Community Development, or a related field.
• Minimum of 3 years of experience in disability support or case management.
• In-depth knowledge of disability services, NDIS principles (if applicable), and support frameworks.
• Strong empathy, patience, and excellent interpersonal and communication skills.
• Ability to work flexibly, including occasional evening or weekend support as needed in George.
• Proficiency in using relevant software for case management and record-keeping.
