The Care Coordinator is responsible for managing and coordinating all aspects of client care to ensure the consistent delivery of high-quality services and client satisfaction. This position oversees client onboarding, staffing, compliance, and daily operational functions. Serving as the primary point of contact for assigned clients, the Care Coordinator ensures that each case is supported with appropriate resources from initiation through long-term service engagement. This role demands proactive communication, strong organizational capabilities, and a detail-oriented approach to case management.
Duties/Tasks to Perform
1. Client Onboarding
- Ensure completion of all onboarding documentation and signed admission agreements prior to service activation
2. Care Coordination and Staffing
- Manage and optimize schedules to ensure 100% of authorized service hours are staffed
- Address aide call-outs, resignations, and client dissatisfaction to prevent care disruptions
- Recruit caregivers to maintain full shift coverage
- Coordinate with vendors and third-party providers as needed
3. Client Case Management
- Maintain up-to-date client records including demographics, preferences, and care plans
- Process authorization updates and adjust staffing accordingly
- Manage discharges with proper documentation and compliance
- Ensure caregiver schedules are accurate and up to date
- Verify and clear all visits daily for timely billing and payroll processing
4. Client Satisfaction and Communication
- Act as the liaison between clients and the company
- Respond to inquiries and concerns in a respectful, timely manner
- Support client retention through follow-ups and proactive service management
5. Compliance and Documentation
- Ensure all client documentation meets regulatory and internal standards
- Document all communications, updates, and incidents in real time
6. Operational Oversight
- Address operational issues such as pending contracts, open shifts, and timesheets
- Foster professional relationships to support lead generation and referrals
Human Relations
- Promote and uphold a professional office culture
- Build empathetic relationships with clients, caregivers, and partners
- Collaborate cross-functionally with internal departments
- Handle sensitive situations with discretion and professionalism
Physical Requirements
- Extended periods of sitting and computer use
- Ability to prioritize tasks under pressure
- Occasional travel for in-person meetings or field visits
Key Performance Indicators (KPIs)
- 100% projected utilization with permanent caregiver staffing
- 90% delivery of authorized service hours
- 100% of discharges/non-admits must be non-preventable
- Minimum client satisfaction score of 4 out of 5
- Timely resolution of all flagged operational items within one hour and same-day closure
Requirements
- Strong organizational and time-management skills
- Excellent verbal and written communication abilities
- Proficiency in Microsoft Office and scheduling software
- Conflict resolution and problem-solving skills
- Ability to manage multiple priorities in a fast-paced setting
- Experience in client services, healthcare coordination, or case management (preferred)
- Familiarity with healthcare authorizations and staffing models (a plus)
- Adaptability to a dynamic, entrepreneurial environment

Friday 12:00PM to 8:00PM and Saturday 8:00AM to 8:00PM
