The Care Coordinator is responsible for managing and coordinating all aspects of client care to ensure the consistent delivery of high-quality services and client satisfaction. This position oversees client onboarding, staffing, compliance, and daily operational functions. Serving as the primary point of contact for assigned clients, the Care Coordinator ensures that each case is supported with appropriate resources from initiation through long-term service engagement. This role demands proactive communication, strong organizational capabilities, and a detail-oriented approach to case management.

Duties/Tasks to Perform:

  • End-to-End Client Onboarding & Compliance: Oversee the complete intake process by ensuring all onboarding documentation and admission agreements are signed prior to service activation; maintain meticulous client records including demographics, preferences, and care plans in accordance with regulatory and internal standards.
  • Strategic Staffing & Schedule Optimization: Manage and optimize caregiver schedules to ensure 100% of authorized service hours are staffed; proactively address aide call-outs and resignations to prevent care disruptions, and recruit new caregivers to maintain full shift coverage.
  • Operational & Financial Administration: Verify and clear all daily visits to ensure timely billing and payroll processing; manage authorization updates, handle discharges with proper documentation, and resolve pending contracts or timesheet issues.
  • Client Advocacy & Liaison Services: Act as the primary liaison between clients and the company, responding to inquiries and concerns in a respectful, timely manner to support high client retention through proactive service management and follow-ups.
  • Real-Time Documentation & Communication: Ensure all communications, service updates, and incidents are documented in real time; collaborate cross-functionally with internal departments to maintain operational transparency and efficiency.
  • Professional Relationship Management: Foster and build empathetic, professional relationships with clients, caregivers, and third-party partners; use these connections to support lead generation, referrals, and vendor coordination.
  • Professional Culture & Ethics: Promote and uphold a professional office culture by handling sensitive situations with high discretion, professionalism, and empathy toward all stakeholders.
  • Dynamic Task Prioritization: Demonstrate the ability to prioritize critical tasks under pressure, addressing immediate operational issues like open shifts while maintaining long-term service quality and compliance.

Schedule: Friday 12:00PM to 8:00PM and Saturday 8:00AM to 8:00PM EST
