Job summary

Rite of Passage in Saint Petersburg, FL is seeking a Transitional Services Coordinator to support youth at Les Peters Youth Academy. You will supervise a caseload of about 13 students, coordinate court hearings, visits, and home passes, and act as the primary liaison among students, families, and placing agencies.

The role requires a bachelor’s degree in psychology or a related field, or equivalent experience in case management, with the ability to pass background checks.

Qualifications

• Have a bachelor's degree in criminal justice, Social Work, Sociology and experience in Case management
• Be able to pass a criminal background check, drug screening, physical, and TB Test.

Responsibilities

• Supervise a caseload of approximately 13 students.
• Assist with facilitating court hearings, Probation Officer visits, and home passes.
• Primary program liaison between student, parents/legal guardian, and placing agency officials.
• Provide monthly progress notes and weekly student updates.
• Coordinate activities with other program staff and participate in assessments and treatment plans.
• Reports to the Director of Student Services.
