Job summary

Preble Street’s Teen Housing Services seeks a Youth Transition Specialist to help youth ages 16–24 transition back into the community from Long Creek YDC and youth involved with the Juvenile Justice System.

You will build trusting relationships, support reentry across housing, education, employment, and family dynamics, and collaborate with a team to provide low-barrier, strengths-based services.

Qualifications

• Bachelor’s degree in social work or related field
• Experience working with youth experiencing homelessness or similar populations
• Understanding and alignment with Preble Street’s mission
• Strong communication, de-escalation, and crisis intervention skills
• Familiarity with community resources
• Strong organizational skills and ability to document services
• Ability to maintain professional boundaries with diverse populations
• Ability to work collaboratively as part of a team

Responsibilities

• Support youth transitioning back into the community from Long Creek Youth Development Center
• Assist youth involved with the Juvenile Justice System with reentry, housing, education, employment, family relationships, and independent living
• Build trusting relationships with youth in the community and in juvenile justice settings
• Collaborate with colleagues and community partners to coordinate services and documentation
