Responsibilities

• Provide client-centered services to at-risk and homeless youth and young adults.
• Assess needs, develop service plans, and connect clients to resources.

• Coordinate care with community partners and social service agencies.
• Maintain timely, professional documentation and HIPAA-compliant records.
• Represent Hale Kipa professionally in all interactions and partnerships.
