Qualifications

• 1–2 years of case management experience with homeless individuals or those with mental health issues.
• Knowledge of harm reduction and trauma-informed care is a plus.
• Ability to manage a flexible schedule, including evenings and weekends.

Responsibilities

• Conduct intake assessments and develop individualized housing stability/service plans.
• Assist clients in securing and maintaining permanent housing.
• Coordinate supportive services including medical and mental health treatments.
