Job summary

St. Catherine’s Center for Children in Albany, NY, seeks a Youth Development Specialist to work within the Prevention program.

Qualifications

• Associate’s degree in human services or education required; Bachelor’s preferred.
• (Key credential required for role.)
• Experience working with multi-issue families and helping them achieve goals.
• (Experience with diverse family situations.)
• Excellent written, organizational, and interpersonal skills.
• (Strong communication and admin skills.)
• Knowledge of community resources incl. schools, law enforcement, churches and clubs.
• (Community network familiarity.)
• Valid New York State driver’s license and reliable vehicle.
• (Travel essential for client transport.)

Responsibilities

• Accept referrals from Albany County DCYF Prevention intake; attend intakes within 24 hours.
• Attend home visits to assess children’s needs for community linkages.
• Link youth to activities aligning with goals and interests.
• Ensure children have a primary care doctor, dentist; ensure schooling status.
• Coordinate services by attending service plan reviews and calls.
• Coordinate with families and clinicians around needs and progress.
• Transport families to appointments and resources.
• Administer NYS CNSS assessment to identify targeted needs.
• Complete progress notes and family plans in NYS OCFS system.
• Advocate for clients with partner agencies in the community.
• Team with clinicians, family advocates, and supervisor on prevention cases.
