Qualifications

• Provides comprehensive case management services for youth and families.
• Caseload of 15 youth both in the facility and in the community aftercare.
• Youth-centered, outcome-focused service philosophy guiding discharge planning.
• Maintain progress notes in Systems like Connections within 5 calendar days.

Responsibilities

• Develop, document, and maintain a comprehensive service plan for youth and families.
• Collaborate with Education Specialist and other providers to discharge planning.
• Schedule family visits within 24 hours of admission and coordinate meetings.
• Engage families throughout assessment and complete required assessments.
• Attend court hearings and prepare reports as required.
