Job summary

The Oaks Academy in Brownwood, Texas, seeks a Case Manager to administer and develop case plans and treatment programs for youth ages 13-17 in a trauma-informed setting. The role reports to the Director of Student Services or Program Director depending on location.

Requirements include a BA/BS in a related field, at least 3 years of youth case management experience, and the ability to pass background checks and screenings.

Qualifications

• BA/BS in a related field.
• 3 years of case management experience with youth/juvenile placement.
• Ability to pass background check, drug screen, physical and TB test.

Responsibilities

• Develop and facilitate completion of case plans and treatment programs as requested by placing agency and students.
• Coordinate with school staff, placement agencies, and program directors to support youth.
• Document progress and respond to changing needs with empathy and professionalism.
