Qualifications

• Must be 18 years of age and able to read and write effectively.

• Must pass background checks and complete required training.

Responsibilities

• Supports the person with personal care, daily routines, and ISP outcomes.
• Assists with meals, grooming, house chores, and community integration.
• Provides safe transportation and accompanies to events, jobs, and appointments.
• Documents daily shift notes and progress toward outcomes in electronic records.
• Upholds confidentiality and follows customer service standards with families and staff.
• Participates in trainings and ensures proper use of agency resources.
