Qualifications

• Minimum High School Diploma or equivalent required.
• Experience in community work and knowledge of community resources.
• Experience working with at-risk youth and families.
• Basic computer knowledge; EHR experience is a plus.
• Strong verbal and written communication skills.
• Reliable transportation, valid driver's license, and current auto insurance.
• Bilingual (Spanish) is a plus.

Responsibilities

• Initiate, organize, plan, develop and implement direct advocacy services to participants and their families.
• Coordinate with families, schools, and services to ensure care.
• Conduct outreach and community engagement to build support networks.
• All service plans will be based on a strength-based approach using the wrap around model.
