Position Title: YHDP Case Manager

Position Summary:

The Youth Homeless Demonstration Program (YHDP) Case Manager provides intensive, individualized case management to youth and young adults (ages 18–24) with dependents who are experiencing or at risk of homelessness. The Case Manager partners with participants to develop individualized housing stability plans that promote long-term self-sufficiency through stable housing, education, employment, life skills, and community connections.

Using a trauma-informed, strengths-based, and youth-centered approach, the Case Manager assists participants in overcoming barriers while supporting their journey toward independence. This position works collaboratively with community partners to ensure youth receive comprehensive, coordinated services that foster long-term success while maintaining compliance with HUD, Continuum of Care (CoC), and Youth Homeless Demonstration Program (YHDP) requirements.

Essential Duties and Responsibilities:
Case Management:
  • Maintain an active caseload of youth participating in the Youth Homeless Demonstration Program.
  • Complete comprehensive intake assessments, eligibility determinations, and individualized service plans with measurable goals.
  • Meet regularly with participants to assess progress, identify barriers, and revise service plans as needed.
  • Provide intensive case management utilizing a Housing First, trauma-informed, and Positive Youth Development approach.
  • Utilize Motivational Interviewing (MI) techniques to engage participants, encourage self-determination, strengthen motivation, and support lasting behavioral change.
  • Provide crisis intervention, de-escalation, and safety planning as needed.
  • Advocate for participants with landlords, employers, schools, healthcare providers, and community agencies.
  • Maintain regular communication with participants to promote engagement and successful program outcomes.
  • Housing Navigation & Home Visits.
  • Assist youth in locating and securing safe, stable, and affordable housing.
  • Coordinate lease applications, move-in assistance, utility connections, and housing stabilization services.
  • Conduct scheduled and unscheduled home visits to assess participant safety, housing stability, program compliance, and ongoing support needs.
  • Monitor housing quality and assist participants in maintaining successful tenancy.
  • Educate participants on tenant rights and responsibilities.
  • Assist participants in resolving landlord concerns and preventing eviction.
  • Coordinate rental assistance and other housing supports in accordance with HUD and YHDP guidelines.
Independent Living & Youth Development:

Provide coaching and education in independent living skills including:

  • Budgeting and financial literacy
  • Meal planning and nutrition
  • Household management
  • Time management
  • Employment readiness
  • Conflict resolution
  • Healthy relationships
  • Goal setting and self-advocacy
  • Accessing community resources
  • Developing natural support systems

Encourage participant leadership, resilience, personal responsibility, and long-term independence through a Positive Youth Development framework.

Resource Coordination:

Coordinate referrals and connections to community resources including:

  • Employment and workforce development
  • Educational opportunities, GED completion, and post-secondary education
  • Mental and behavioral health services
  • Substance use treatment
  • Medical and dental care
  • Transportation
  • Childcare (when applicable)
  • Legal assistance
  • Financial Literacy Resources
  • Food and other basic needs
  • Public benefits and community assistance programs
  • Collaborate with community providers to ensure participants receive coordinated, wraparound services.
  • Assist in bridging YHDP participants to other programs within The Salvation Army
  • Assist with all Salvation Army holiday and special events (Christmas Assistance, Dress-A-Child, Red Kettle Kickoff, Daffodil Tea, Back to School, Coat Drive, etc.)
  • Other duties as assigned by supervisor or Corps Officers
Documentation & Data Management:
  • Maintain accurate, timely, and confidential participant files.
  • Enter all required case notes, assessments, service plans, progress updates, referrals, and program activities into Wellsky within agency-established timelines.
  • Accurately enter participant data into the Homeless Management Information System (HMIS)to ensure compliance with HUD, YHDP, and Continuum of Care reporting requirements.
  • Maintain complete documentation supporting participant eligibility, housing placement, rental assistance, inspections, and program outcomes.
  • Monitor participant progress and document measurable outcomes.
