Job summary

Los Altos Post-Acute is seeking a Treatment Nurse to care for residents with skin concerns and other medical assessments under the supervision of the DON and attending physicians. You will help modify treatment plans and document care in resident charts while coordinating with the care team.

The role requires a California LVN license, CPR/First Aid certification, and strong communication skills. You will work on site at our Los Altos, CA facility, delivering compassionate, patient-centered care

Qualifications

• Current California LVN license required.
• CPR and First Aid certification required.
• Strong verbal and written communication abilities.
• Ability to remain calm and professional in all work-related situations.

Responsibilities

• Provides treatment and therapeutic services per physician orders.
• Consults with the DON and care team to develop treatment plans.
• Examines residents and medical records, recommending physician referrals when needed.
• Assesses skin conditions and documents stages and issues on admission or readmission.
• Performs weekly skin exams and records results and treatment orders.
• Ensures decubitus ulcers receive proper prophylaxis and treatment.
• Completes medical forms, reports, and charting.
• Collaborates with interdisciplinary team on wound care and diet changes.
• Educates residents and families on prevention and health maintenance.
• Orders medications/equipment as required and ensures timely care.
• Audits and updates treatment sheets and charting; attends meetings.
