Job summary

AdventHealth Shawnee Mission in Kansas seeks an experienced RN specialized in wound, ostomy, and continence care to join our team. You will assess patients, develop care plans, and document in the EMR while coordinating with physicians and staff to deliver patient-centered care.

The role requires ADN with preference for BSN, 1+ year acute care and wound/ostomy experience, and a strong focus on safety, infection control, and education for patients and staff.

Responsibilities

• Conduct comprehensive patient assessments for wound, ostomy, and continence needs.
• Develop and document individualized care plans based on assessments.
• Maintain accurate documentation in the EMR.
• Administer treatments and medications safely and monitor responses.
• Provide direct wound, ostomy, and continence care.
• Educate patients and families on wound care and self-care strategies.
• Guide nursing staff on wound, ostomy, and continence care.
• Collaborate with physicians and interdisciplinary teams to coordinate care.
• Monitor patient progress and revise care plans as needed.
• Respond to urgent needs and participate in on-call coverage, including weekends/holidays.
• Assess educational needs of staff and support competency in specialty care.
• Incorporate evidence-based practice into patient care and quality initiatives.
• Communicate concerns and recommendations to Clinical Nurse Manager and team.

Job Description

Work Hours/Shifts:

• 4 Days/Week
• 10-hour shifts- 0700 - 1730
• Would be visiting our SOP and LCC locations frequently.
• There is no on call but weekends may be required.

Job Responsibilities

• Conduct comprehensive patient assessments to determine physical, mental, social, and spiritual needs related to wound, ostomy, and continence care.
• Develop and document individualized care plans based on patient assessments, selecting appropriate interventions and supplies.
• Maintain accurate and timely patient documentation in the electronic medical record.
• Administer treatments and medications safely while monitoring patient response and adjusting care plans as appropriate.
• Provide direct wound, ostomy, and continence care that promotes healing, patient comfort, and optimal outcomes.
• Educate patients and families on wound care, ostomy management, continence strategies, self-care techniques, and lifestyle modifications.
• Provide education and clinical guidance to nursing staff and other healthcare professionals on wound, ostomy, and continence care.
• Collaborate with physicians, nurses, and interdisciplinary team members to coordinate comprehensive, patient-centered care.
• Monitor, evaluate, and document patient progress, revising care plans based on patient response and clinical outcomes.
• Respond to urgent patient care needs and participate in on-call coverage, including weekends and holidays, as required.
• Assess the educational needs of nursing staff and support competency in skin integrity, wound prevention, and specialty care.
• Incorporate evidence-based practice, current research, and clinical guidelines into patient care and quality improvement initiatives.
• Communicate patient care concerns, barriers, and recommendations to the Clinical Nurse Manager and interdisciplinary team to support safe, effective, and coordinated care.

• Basic Life Support (BLS) certification. (Preferred)
• Certified Wound Care Nurse (CWCN). (Preferred)
• Certified Wound, Ostomy, and Continence Nurse (CWOCN). (Preferred)
• Advanced Practice Registered Nurse (APRN). (Preferred)
