Job summary

Moorestown Visiting Nurse Association in New Jersey is seeking a Wound, Ostomy and Continence Nurse (WOCN) to serve as an expert consultant to the healthcare team and to home care patients.

The WOCN will provide wound, ostomy and continence services, educate patients and caregivers, and collaborate with physicians and the family across our service area. The position requires independent clinical judgment and adherence to best practice standards in a home health setting.

Qualifications

• Interpersonal and communication skills to educate patients and caregivers.
• CPR certification and RN licensure required.

Wound, Ostomy, and Continence RN (WOCN)

We are currently seeking a Wound, Ostomy and Continence Nurse to function as an expert consultant to the healthcare team and to our home care patients. The Wound, Ostomy and Continence Nurse will provide clinical wound, ostomy and continence services to patients with functional limitations that affect their ability to carry out activities of daily living. The Wound, Ostomy and Continence Nurse will also provide direct care, support, and education in the home under the direction of the attending physician and in collaboration with the family other disciplines. Assignments require considerable independence of professional judgment and are carried out in accordance with best practice standards and established policies, procedures, rules and regulations of the agency.

Essential Duties and Responsibilities:

• Assess referred patients for clinical services.
• Develop and educate patients and their care givers on the plan of care.
• Thoroughly complete all required documentation.
• Possess excellent interpersonal, communication and time management skills.
• Willingness to travel within our service area.

Education and Experience:

• Licensed as a RN by the State of New Jersey.
• Certification for CPR.
• At least 2 years of acute care experience.
• Certified WOCN or WCC
