Job summary

Chandler Creek Post Acute in Greer, South Carolina, is seeking an experienced Wound Care Nurse (LPN) to join our clinical team. You will provide specialized skin and wound care to residents under physician oversight, supporting healing and quality outcomes.

This full-time role requires an active SC LPN license, CPR, and the ability to work 12-hour shifts. Experience with PointClickCare (PCC) is preferred, and prior wound care experience is strongly valued.

Qualifications

• Active, unencumbered LPN license in South Carolina.
• Graduate of an accredited Practical Nursing program.
• Current CPR certification required.
• Ability to work 12-hour shifts.
• Experience with PointClickCare (PCC) preferred.
• Prior wound care experience strongly preferred.
• Strong clinical judgment and attention to detail.

Responsibilities

• Provide direct wound care and skin treatments in accordance with physician orders.
• Assess, monitor, and document wound progress and treatment effectiveness.
• Assist in developing and modifying individualized care plans.
• Collaborate with the interdisciplinary team to ensure optimal outcomes.
• Educate staff on proper wound care techniques and prevention strategies.
• Maintain accurate and compliant clinical documentation.
• Ensure adherence to all regulatory, safety, and infection control standards.

Position Summary

As a Wound Care Nurse (LPN), you will be responsible for providing specialized skin and wound care to residents under the direction of the physician, Director of Nursing, and/or Medical Director.
