Job summary

Care Pavilion is seeking a Wound Care Nurse to join our expert team in Philadelphia. The Wound Care Coordinator collaborates with the Director of Nursing, physicians, and interdisciplinary team to ensure consistent quality care for residents with wounds and to educate staff on prevention and treatment principles.

The role involves assessing skin conditions, completing Braden Scales, implementing and evaluating care plans, and maintaining detailed documentation.

Qualifications

• Graduate of accredited school of nursing.
• Current, active license as a Registered Nurse in state of employment.
• Wound Care Certification required. Must understand the principles of wound prevention and treatment.
• Active CPR Certification is required.
• Three years of experience providing direct care to residents in a long-term care setting.

Responsibilities

• Assesses skin condition on new admissions and readmissions using the standardized assessment tool.
• Completes Braden Scales as needed.
• Implements treatment plan on residents as appropriate and administers treatments.
• Implements and evaluates the plan of care reflecting appropriate nursing and medical diagnosis, goal development, outcome criteria and target dates.
• Keeps resident plans of care updated reflecting changes in condition and responses to treatment or education.
• Documents on residents receiving treatments and reviews previous documentation.
• Participates with the interdisciplinary team and wound care committee; completes the weekly wound report.

The Wound Care Coordinator communicates and collaborates closely with the Director of Nursing, physicians, Unit Managers, and other members of the interdisciplinary team to provide consistent quality care to those residents with wounds. Works closely with the Director of Staff Development/Infection Prevention and Control Officer and Quality Improvement team to ensure that all the nursing staff is educated in the principles of wound prevention and treatment. Implements treatment modalities as assigned.

Job Duties:

• Assesses skin condition on new admissions and readmissions using the standardized assessment tool.
• Completes Braden Scales as needed.
• Implements treatment plan on residents as appropriate and administers treatments.
• Implements and evaluates the plan of care reflecting appropriate nursing and medical diagnosis, goal development, outcome criteria and target dates. Keeps resident plans of care updated reflecting changes in condition and responses to treatment or education.
• Appropriately documents on residents receiving treatments, reviewing previous documentation.
• Complete Initial/Weekly Skin Impairment Form
• Participates with the interdisciplinary team, including the Wound Care Team in Weekly Wound Rounds and is part of the Wound Committee; completes the weekly wound report.
• Participates in the weekly Skin and Nutrition Committee Meeting; participates in the weekly wound conference call; attends quarterly Wound Care Coordinators Meeting.
• Works with restorative nursing to make sure staff is following through with active measures to prevent skin breakdown and that residents who are at risk for skin breakdown are identified.
• Works closely with Nursing, Restorative Nursing, Physical and Occupational Therapy to ensure that residents have received the appropriate pressure redistribution surfaces / measures, and proper application and monitoring of devices.
• Documents and informs residents, responsible parties and Nursing Administration of any changes in skin integrity, deterioration of wounds, changes in care and treatment of wounds.
• In-services staff as needed in, including but not limited to, the following areas: Treatment modalities, pressure redistribution surfaces, positioning devices, nutrition, wound care documentation, classification of wounds, wound measurement, staging of wounds, and care planning.

Job Qualifications:
