Job summary

Dumont Center for Rehabilitation & Nursing Care seeks a Wound Care Nurse to oversee the follow-up of residents with wounds of all kinds. The role emphasizes early wound assessment, treatment coordination, and ongoing plan updates.

Qualifications

• RN with at least 2 years of experience in wound care or management.
• Strong coordination with units, physicians, and other departments.
• Proficient in documenting wound progress and treatment responses.

Responsibilities

• Check new admissions/readmissions for wounds.
• Coordinate with unit managers and nursing supervisors for nosocomial wounds.
• Ensure treatment orders are in place.
• Monitor response to treatment orders.
• Weekly wound rounds with wound doctor.
• Initiate individual wound care plans.
• Initiate and update weekly wound tracking and distribute to departments.
• Update and evaluate wound care plans quarterly or on significant changes.
• Inform next of kin of residents with wounds at admission/admission and during care.
• Ensure residents have specialized mattresses and off-loading devices.
• Coordinate with Rehab for positioning devices.
• Coordinate with Dietitian for nutritional needs.
• Coordinate with Attending Doctors on blood values and changes in orders.
• Coordinate with Unit Managers to ensure treatments and devices are in place.
• Make weekly audits for off-loading devices and special mattresses and report findings to DNS/ADNS.
• Assist Nursing Department as needed under DNS/ADNS directive.
