Job summary

Bluebird Wellness & Rehabilitation seeks a registered nurse to coordinate the centre’s wound team and oversee the skin management program. The role includes leading rounds, care planning, documentation, and collaboration with an interdisciplinary team.

Responsibilities include gathering pressure ulcer data, guiding weekly wound team rounds, and ensuring Braden risk assessments and preventive measures are implemented as directed.

Qualifications

• Licensed RN, LVN or LPN in state requirements

Responsibilities

• Identify and maintain effective methods to gather pressure ulcer and skin alteration information
• Coordinate and lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation
• Collect, analyze and provide pressure ulcer information to nursing staff and other healthcare practitioners
• Investigate and identify center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions
