Qualifications

Responsibilities

• Promotes patient and family centered care in a healing environment and educates on illness management and home care needs.
• Participates in activities that promote patient safety, quality and regulatory compliance.
• Develops and documents care plans with goals in collaboration with the interprofessional team and patient/family.
• Uses nursing assessment to evaluate physical, psychosocial, and emotional needs and communicates plans across the continuum of care.
• Maintains safety as a core function and adheres to standards of nursing care.
