Job summary

Topaz Fiscal Services seeks an experienced nurse to lead the skin care program in a resident facility. You will assess skin conditions weekly, document findings, and coordinate with physicians and staff to develop and update care plans.

The role requires an RN license (LVN/LPN by exception) with at least 3 years of related experience, strong organization, and the ability to educate residents, families, and staff on wound prevention and healing per F314.

Qualifications

• RN license preferred; LVN/LPN by exception.
• Minimum 3 years related experience.
• Must be in good standing and licensed by the State.
• Relate positively to residents and families; cooperate with staff.
• Must be organized and efficient to manage the skin program.

Responsibilities

• Assess residents weekly to evaluate skin condition and document.
• Measure skin areas weekly and document.
• Initiate and update skin care plans.
• Report and transcribe physician orders for skin issues.
• Perform skin treatments per physician orders and standards.
• Coordinate with physician, residents, and staff; report status.
• Chair Skin Management meeting weekly and guide interventions.
• Monitor equipment use and maintain records; evaluate new devices.
• Follow facility skin protocol and F314 guidelines.
• Provide weekly skin logs and education to residents, families, staff.
• Participate in Quality Improvement activities.
