Job summary

BrightStar Care in Humble and Kingwood, TX is seeking an experienced Registered Nurse with a strong wound care background to provide specialized home-based services. You will perform wound, skin, ostomy, and continence care in patients' homes, with comprehensive assessments and care planning.

The role emphasizes independent practice, excellent communication and documentation, and collaboration with physicians and caregivers.

Qualifications

• RN license and strong wound care background.
• Excellent communication and documentation skills.
• Comfortable working independently in home health.
• WOCN certification preferred but not required.

Responsibilities

• Provide specialized wound, skin, ostomy, and continence care.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Perform wound treatments and dressing changes and monitor healing progress.
• Educate patients and caregivers on prevention and ongoing wound management.
• Collaborate with physicians and interdisciplinary care teams.
