Job summary

The Phoenix Rehabilitation and Nursing Center in New York is seeking a Wound Care Registered Nurse (RN) to join our team as a Full Time staff member. The role focuses on long-term wound care in a nursing and rehab setting and includes weekly wound documentation and ensuring treatments are carried out as directed.

Requirements include a valid NY RN license, active CPR certificate, and wound care certification if possible. Strong communication, self-motivation, and teamwork are essential.

• CPR certificate must be current.

• Responsible for accurate and timely documentation of wounds in the nurse's notes at least weekly.
• Responsible for all treatments required by licensed nurse on your shift and ensuring the wound care treatments are carried out at other times.
• Evaluate high risk patients for pressure reduction interventions methods
• Conduct body assessments and documentation on all new admissions per policy
• Round with doctors weekly to assist with assessing residents

The Phoenix Rehabilitation and Nursing Center is looking for a talented and hard-working Full Time Wound Care Nurse to join their team! The ideal candidate will possess a strong knowledge in long term wound care in a nursing & rehab facility.

Wound Care Registered Nurse (RN)Duties Include but are not limited to:

Responsible for accurate and timely documentation of wounds in the nurse's notes at least weekly
