Job summary

Excelsior Care Group in Nassau County, NY is seeking an experienced Wound Care RN to join our skilled nursing team. You will assess residents with wounds, collaborate with physicians on individualized treatment plans, and lead weekly wound rounds to monitor progress.

Qualifications

• Active New York State RN license.
• Wound Care Certification and Infection Control Certification required.
• Minimum 1 year nursing home or long-term care experience preferred.

Responsibilities

• Assess residents with wounds and monitor new wounds.
• Collaborate with physicians to create wound care plans.
• Lead weekly wound rounds and document measurements.
• Maintain weekly wound tracking reports and report non-healing wounds.
• Track and analyze weekly decubitus ulcer statistics.
• Provide wound care training to nursing staff.
• Prepare QA reports to support regulatory compliance.
• Observe dressing changes and ensure staff competency.
• Educate residents and families on wound care.
• Attend ongoing wound care in-services to stay current with industry standards.

We are seeking an experienced and compassionate Wound Care RN to join our skilled nursing team. This role is ideal for a clinically strong nurse who is passionate about wound management, resident outcomes, and quality care.

Key Responsibilities:

• Assess residents admitted with wounds or those who develop wounds during their stay
• Collaborate with physicians to develop and implement individualized wound care treatment plans
• Lead and participate in weekly wound rounds, including wound measurements and documentation
• Complete and maintain weekly wound tracking reports; promptly report non-healing wounds to physicians
• Track, analyze, and distribute weekly decubitus ulcer statistics
• Provide in-service education and training to nursing staff on wound care best practices
• Prepare quarterly Quality Assurance (QA) reports to support regulatory compliance
• Observe and evaluate dressing changes to ensure staff competency
• Educate residents and families on wound care as appropriate
• Attend ongoing wound care in-services to stay current with industry standards
