Qualifications

• Current, valid LPN or RN license in the state of Tennessee.
• Wound Care Certification is required or preferred.
• Prior wound care experience in a long-term care or skilled nursing setting is required.

Responsibilities

• Conduct thorough assessments of wounds, pressure injuries, ulcers, surgical sites, and other skin conditions.
• Develop and implement individualized care and treatment plans based on findings and orders.
• Perform wound cleansing, dressing changes, and advanced wound therapies per protocols.
• Monitor for infection or complications and report changes promptly.
• Document assessments and progress in compliance with policies and regulations.
• Educate residents, families, and staff on wound prevention and skin care techniques.
