Job summary

Port Washington Post Acute in Bremerton, WA is seeking a Wound Care Nurse (RN or LPN) to lead our skin and wound care program full-time. You’ll assess, treat, and prevent pressure injuries, coordinate with physicians and the DON, and educate residents, families, and staff on best practices.

This specialty role focuses on wound care with direct impact on healing outcomes, working closely with the Medical Director and nursing leadership to ensure quality care.

Qualifications

• Must have a current, active Washington RN or LPN license.
• Experience in long-term care, skilled nursing, or post-acute settings.
• Wound care experience with assessment, staging, and treatment.
• Wound care certification (WCC, CWCN, or similar) preferred but not required.

Responsibilities

• Lead the facility’s skin and wound care program, including assessment, treatment, and prevention of pressure injuries and other skin conditions.
• Perform wound assessments, staging, and measurements, and track healing progress.
• Partner with physicians, the Medical Director, and the Director of Nursing on treatment plans and referrals.
• Collaborate with the interdisciplinary team on care plans and skin-related assessments.
• Train nursing staff and CNAs on skin care, prevention techniques, and wound care best practices.
• Educate residents and families on skin care, especially before discharge home.
• Keep wound care supplies and treatment products stocked.
• Maintain accurate, timely documentation and support state survey readiness.
• Advocate for residents’ dignity, rights, and quality of care.
