Job summary

South Shore Rehabilitation in Chicago is seeking a dynamic Wound Care Nurse to provide high-quality wound care, implement preventative measures, and ensure compliance with federal and state regulations as well as facility policies.

Qualifications

• Active Nursing License as required by the state.
• Current CPR certification.
• Minimum two years of RN or LPN experience.
• Ability to lift and move patients up to 75 lbs.
• Strong verbal and written communication in English.
• Ability to cope with mental and emotional stress of the position.
• Strong attention to detail and ability to prioritize wound care needs.

Responsibilities

• Utilize the nursing process for assessment, planning, implementation, and evaluation of wound and skin care.
• Conduct regular wound care rounds with timely documentation of resident skin conditions.
• Assess resident skin integrity and relate medical history to wound care.
• Implement preventative wound care strategies in care plans.
• Prioritize nursing interventions for wound management.
• Communicate with physicians and interdisciplinary teams regarding wound treatment needs.
• Initiate nutritional interventions to support wound healing.
• Perform direct resident care in compliance with facility policies and procedures.
• Ensure residents with incontinence receive appropriate pericare and preventative treatments.
• Maintain a safe, clean work environment adhering to infection control protocols.
