Job summary

Sweetwater Care in California seeks an experienced LVN with nursing management responsibilities to oversee patient care in a long-term care setting. The role requires licensure in California and prior management exposure, with preference for SNF/LTC experience and familiarity with Point Click Care charting.

The ideal candidate will coordinate care, educate families and patients, and ensure timely medication and treatment administration in accordance with policy and physician orders.

Qualifications

• Currently LVN licensed in California.
• One-year nursing management experience preferred.
• Experience in SNF/LTC settings preferred.

Responsibilities

• Provides nursing care within scope of practice per standards, orders, and guidelines.
• Notifies departments of patient changes and admissions/discharges.
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Might make rounds with physicians as needed.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications
