JESSICA WONG NURSE PRACTITIONER is seeking a Licensed Vocational Nurse (LVN) to join our wound care team. You will assess, treat, and manage wounds, monitor progress, and communicate changes to the supervising RN or Provider.

The role includes administering dressings and topical agents, documenting care, and using EMR systems like WoundAlly while upholding HIPAA standards. Travel to homes and facilities in the LA region and surrounding areas is required.

Qualifications

• Must hold California LVN license.
• Wound care experience preferred.
• Excellent interpersonal and client care skills.
• Valid driver’s license and auto insurance required.
• First aid/CPR certification required.

Responsibilities

• Coordinate with colleagues to assess, plan, implement, and evaluate patient care plans.
• Administer wound care treatments, dressings, and topical agents with aseptic technique.
• Document changes in patient conditions and report to supervisor.
• Maintain accurate records of patient care and discuss observations with supervisor.
• Perform comprehensive wound assessments including measurement and documentation of healing progress.
• Document all patient interactions in EMR/EHR systems such as WoundAlly, adhering to HIPAA.
• Educate patients and families on wound management, nutrition for healing, and lifestyle changes.
• Travel to patients' homes and facilities across Los Angeles County, Orange County, Inland Empire and surrounding areas.
