Job summary

Hartford HealthCare at Home is seeking a Wound Care LPN to join our home-based wound care program in Connecticut. The role partners with RNs, CWOCN, and the manager to deliver outcome-based wound care and cost-effective treatments in patients’ homes.

The LPN will perform wound care, participate in care planning, and help maintain clinical care standards while supporting staffing and program development. A CT-approved LPN program is required.

Qualifications

• Wound care expertise required for home-based patients.
• Experience in clinical settings, preferably in wound/ostomy care.
• Ability to work as part of a wound care team and coordinate with RNs/CWOCNs.

Responsibilities

• Perform daily wound care and treatments in patient homes.
• Collaborate with Wound Team RN, CWOCN, and Manager of Specialty Services to meet program goals.
• Participate in staffing decisions to meet service demands and ensure quality outcomes.
• Support training, onboarding, and professional development for wound program staff.
• Contribute to performance improvement initiatives (H3W) and clinical education programs.
