Job summary

Green Hills Center for Rehabilitation and Healing is seeking a Wound Care Nurse to provide primary skin care to residents under medical direction, focusing on treatment and therapy of skin disorders. You will collaborate with physicians, the Nursing Supervisor, and the Medical Director to modify regimens as needed and ensure high-quality resident care.

The role includes administrative duties, documentation, and participation in policy implementation.

Qualifications

• Provide care for skin disorders and wound management under medical direction.
• Collaborate with Nursing Supervisor, Medical Director, and Care Planning Team.
• Perform routine assessments and adjust treatment plans as needed.

Responsibilities

• Confer with the Director of Nursing and licensed nursing staff regarding dermatologic disorders.
• Consult with Care Planning Team for assessments and skin care plans.
• Identify when referrals to physicians are needed and initiate consultations.
• Complete medical forms, reports, and charting related to skin care treatments.
• Implement and maintain skin care policies and procedures and communicate with residents and families.
