POSITION SUMMARY

The Wound Care Coordinator reports to and receives general direction from the Director of Nursing and is responsible for delivering quality wound care and preventative measures to maintain skin integrity to assigned residents in compliance with applicable federal and state guidelines and established policies and procedures.

ESSENTIAL JOB FUNCTIONS

1. 1. Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the resident’s wound and skin integrity
2. 2. Assesses each resident; collect and document the resident’s current skin status including medical history. Complete and/or review the Admission Assessment record. Make accurate and ongoing assessments of the resident’s wounds/risk of wound development and respond accordingly
3. 3. Sets priorities for nursing action for wound care and prevention in a logical sequence according to the resident’s needs and formulate a Plan of Care. The Plan of Care is initiated on resident admission based on all aspects of the initial assessment. The Plan shall include physiological, psychosocial, and environmental factors
4. 4. Evaluates: Performs ongoing assessments and revise based on new data and resident condition
5. 5. Documentation: Documents accurate and ongoing assessment of the resident’s skin status
6. 6. Communicates with physician’s regarding resident wound needs, the nursing assessment and recommendations as needed. Conduct weekly wound meetings to collaborate with other departments to ensure the resident’s wellbeing
7. 7. Initiates nutritional interventions to ensure that residents with wounds are provided with nutrition to promote wound healing
8. 8. Performs direct resident care utilizing established procedure, policies, and standards Administer treatments and procedures in accordance with the physician’s orders
9. 9. Ensures that residents are medicated prior to treatments when appropriate
10. 10. Observes and documents resident’s response to treatment at a minimum weekly and as needed.
11. 11. Ensures proper disposal of treatment supplies
12. 12. Ensures adequate levels of treatment ointments, supplies and equipment are available at all times. Report any needs to the Director of Nursing.
13. 13. Handles any emergency situations in a prompt, precise and professional manner
14. 14. Maintains the comfort, privacy, and dignity of each resident while performing treatments
15. 15. Interacts with residents in a manner that displays warmth and promotes a caring environment

1. 16. Maintains a comprehensive understanding and adherence to all aspects of resident rights including the right to be free of abuse. Promptly report to the Administrator all incidents or evidence of resident abuse or violation of resident’s rights
2. 17. Assures residents have call lights within reach and answer call lights promptly and courteously
3. 18. Assists with orienting residents and families to the wound care process upon admission
4. 19. Reports the presence of pressure ulcers to the Floor Nurse and Certified Nursing Assistants
5. 20. Promptly reports injuries of unknown origin to the Floor Nurse and Director of Nursing, including skin tears
6. 21. Communicates and interacts effectively, courteously, and tactfully with residents, visitors, family members, peers, and supervisors
7. 22. Thoroughly washes hands before, during and after treatments provided to residents
8. 23. Conducts rounds frequently to ensure that residents are being turned and repositioned timely. Disciplines non-compliant staff accordingly with consultation from the DON
9. 24. Conducts rounds frequently to ensure that residents with incontinence are receiving peri care and preventative ointments timely. Discipline non-compliant staff accordingly with consultation from the DON
10. 25. Responds to inquiries or requests from residents and family members in a prompt and courteous manner
11. 26. Promotes positive public relations with residents, family members and visitors
12. 27. Maintains the confidentiality of all residents' care information
13. 28. Spot checks shower sheets for proper evaluation of resident’s skin integrity. Discipline noncompliant staff accordingly.
14. 29. Honors the resident’s refusal of treatment request. Documents and promptly reports such requests to the Director of Nursing and Social Services
15. 30. Maintains current knowledge of resident condition through the maintenance of the Pressure Ulcer and Non-Pressure Ulcer Weekly Reports.
16. 31. Transcribes, clarifies and implements physician orders
17. 32. Ensures that residents receive required support surfaces for prevention and healing of wounds. Disciplines staff non-compliance accordingly
18. 33. Conducts Wound Care Rounds with the facility’s Wound Specialist and implement all recommendations. Update the residents Plan of Care accordingly
19. 34. Completes unavoidable documentation in accordance with the resident’s physician for residents that develop pressure ulcers or worsening pressure ulcers within the facility.
20. 35. Performs duties in a safe manner in accordance with established policies and procedures
21. 36. Practices efficient and no waste use of supplies in accordance with established policies and procedures
22. 37. Performs all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves and proper disposal of soiled material
23. 38. Actively participates in fire, safety, and disaster preparedness drills
24. 39. Maintains safe working conditions and practices and follow established safety regulations in the use of equipment and supplies

EDUCATION/EXPERIENCE

1. 1. Graduate of an accredited school of Nursing
2. 2. Must possess a current and unencumbered Licensed Professional Nurse or Registered Nurse License in the state the facility resides in.

PHYSICAL DEMANDS

1. 1. Must be able to push, pull, move, and/or lift a minimum of 50 pounds.
2. 2. Must be able to stand and walk for long periods of time.
3. 3. Must be able to reach, stoop, kneel, bend, lift, and move intermittently throughout the workday throughout the facility
4. 4. Must be able to work outside of normal work hours including nights, holidays, and weekends as needed to meet facility and position demands
5. 5. Must be free of disease that may be transmitted in the performance of job responsibilities during the stage of communicability
6. 6. Must be able to relate and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility.

WORK ENVIRONMENT

The Wound Care Coordinator works throughout the facility and at times beyond normal duty hours, on weekends and holidays when necessary and will interact with residents, family members, and staff under all conditions and circumstances. May have exposure to infectious waste, diseases, and conditions including but not limited to blood borne pathogens.
