Job summary

Infinity Healthcare Consulting is seeking a Wound Care Coordinator (RN or LPN) in Illinois to deliver high-quality wound care and skin integrity management for residents in accordance with federal/state guidelines and facility policies.

The role requires nursing licensure (RN or LPN) in the state and focuses on assessment, documentation, plan of care, coordination with physicians, and ensuring patient comfort and safety while maintaining dignity.

Qualifications

• Graduate of an accredited school of Nursing.
• Must possess an unencumbered RN or LPN license in Illinois.

Responsibilities

• Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation and evaluation of residents' wound and skin integrity.
• Collecting and documentation of residents' skin status including medical history.
• Wound care prevention and prevention through plan of care.
• Sets priorities for nursing action for wound care and prevention.
• Communicates with physicians regarding resident wound needs.
• Initiates nutritional interventions to ensure that residents with wounds are provided nutrition to promote wound healing.
• Performs direct resident care utilizing established procedure, policies, and standards.
• Administer treatments and procedures in accordance with the physician's orders.
• Maintains the comfort, privacy, and dignity of each resident while performing treatments.
• Communicates and interacts effectively, courteously, and tactfully with residents, visitors, family members, peers, and supervisors.
• Conducts rounds frequently to ensure that residents with incontinence are receiving pericare and preventative ointments timely. Discipline non-compliant staff accordingly with consultation from the DON.
• Performs duties in a safe manner in accordance with established policies and procedures.
• Performs all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves and proper disposal of soiled material.

POSITION SUMMARY:

The Wound Care Coordinator (RN or LPN) reports to and receives general direction from the Director of Nursing and is responsible for delivering quality wound care and preventative measures to maintain skin integrity to assigned residents in compliance with applicable federal and state guidelines and established policies and procedures.

EDUCATION/EXPERIENCE:

• Graduate of an accredited school of Nursing
• Must possess a current and unencumbered Licensed Professional Nurse (LPN) or Registered Nurse (RN) License in the state the facility resides in
