Zusammenfassung

The Women's Health Care Coordinator role in Monticello, AR is a full-time position within Mainline Health Systems, Inc. You will coordinate care with providers outside the system, support patient enrollment in Breast Care programs, and help maintain our Patient Centered Medical Home model of care.

Responsibilities include scheduling, referrals, documentation into the EMR, patient communication, and participation in daily huddles and monthly staff meetings.

Qualifikationen

• High School Diploma or GED is required. Bachelor of Social Work or health care related field is preferred.
• Ability to communicate clearly, takes direction, and work well with a team but also self motivated.
• Detail oriented.
• Ability to navigate and utilize electronic medical records.
• One year of experience in medical field/setting.

Aufgaben

• Make appointments, send referrals to outside providers, complete prior authorization if needed, send Medicaid Referral Form for Medicaid patients, notify patient of appointment, follow-up on appointment, request records, and scan records into EMR. Document all information in the open outgoing referral.
• Breast Care
• Assist patients in enrollment of Breast Care and entering results into ADH website.
• Interacting with patients and staff on a daily basis.
• Communication with management in regards to any related issues.
• Notification of patients regarding missed appointments,
• Process requests from patients and staff via electronic messages.
• Additional duties or responsibilities as deemed necessary.
• Participates in monthly clinical staff meetings
• Participate in daily huddles and maintain an active role within the care team
• Serve as part of the care team; following and implementing workflows that support and sustain MHSI Patient Centered Medical Home model of care.
• Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI.
• Completion of continual job training processes set forth by Mainline Health Systems, Inc .
