Qualifications

  • Current state licensure, as applicable, obtained prior to or upon supervision hours.

Responsibilities

  • Perform psychosocial assessments and develop care plans with the care team.
  • Provide clinical counseling, brief therapy, and referrals as needed.
  • Coordinate care addressing social determinants and community resources.
  • Deliver individual, group, and family therapy as indicated.
  • Lead care transitions and advocate for patient rights and access to services.
  • Collaborate with providers, insurers, and community partners to ensure quality care.
  • Monitor quality metrics and ensure policy adherence.
