Job summary

Corewell Health West WOC is seeking a skilled RN to provide wound, ostomy, and continence care. The role focuses on teaching patients and families, planning individualized care, and coordinating discharge processes to ensure safe home transitions.

You will collaborate with a multidisciplinary team, monitor quality indicators, and participate in policy development to advance patient outcomes. A Michigan RN license is required, with competency-based orientation and 1+ year of clinical experience

Qualifications

Bachelor's degree in nursing required.
RN license in Michigan upon hire.
Experience teaching patients, families, and other health professionals preferred.
1 year of relevant clinical nursing experience preferred.
Competency-based orientation completed required.

Responsibilities

Assess and monitor patients’ physical needs related to ostomy, wound, and continence.
Develop and maintain individualized plans of care.
Assist in discharge planning and transitions to home or next care setting.
Assess educational needs of patients and caregivers; provide teaching and referrals.
Collaborate with nurses, providers, and departments to improve care delivery and outcomes.
Develop and monitor quality indicators with the nursing quality department.
Participate in policy development and evidence-based practice initiatives.
Support nursing research and promote nurse-sensitive patient outcomes.
