The Vision of Person Directed Living

Person Directed Living (PDL) shapes and informs the ways we care for and serve the sisters of the Holy Cross in their living communities and for one another as a team. Team members will create an environment that is life-giving, respectful, safe and clinically appropriate for Sisters honoring the sisters’ dignity and personhood; respectful and collaborative with each other as teammates.

PDL is intentionally integrated into both clinical care that the sisters receive as well as assisting their engagement in daily living and the life of their local community. Team members will develop relationships with the sisters and among themselves so they may deeply know and support everyone in the community. Robust, respectful communication (verbal and non-verbal) is the PDL standard.

As their ability dictates, Sisters are encouraged and supported in making individual choices about their daily routines and communal life through creative and thoughtful engagement by team members. PDL demands a delicate balance between the sisters’ abilities and the team’s mandate for safety and dignity, calling upon each team member to assist Sisters with innovative solutions, and bringing joy and hope to every member of the community, Sisters and team members alike.

Main Function:

Works within their scope of practice in the primary responsibility and accountability of providing, assisting and delegating in the provision of quality comprehensive nursing care to the Sisters. Assists in creating a collaborative care environment in cooperation with Nurse Coordinators, and Care Partners. Takes an active role to provide clinical training in-the-moment.

Participates in the On Call Scheduling rotation.

Communication and Relationship Skills

ESSENTIAL FUNCTIONS
  • As the primary whole house supervisor, you would be responsible to initiate appropriate measures in an emergency. Directs movement of all sisters and staff present. Ensures and provides safety training for Nurses, Care Partners and Sisters. Works on all floors assuring that Care Partners and nurses are provided oversight and support throughout the entire shift.
  • Understands and complies with HR policies and procedures developed by Sisters of the Holy Cross. Adheres to all Resident Services and Nursing policies and procedures.
  • Responsible for safety and welfare of the Sisters through proper implementation of nursing procedures. Routinely observes the care given by staff and makes suggestions accordingly.
  • Follows Universal Precautions and Infection Control guidelines.Works closely and collaboratively with Care Partners and nurses, providing mentoring in clinical areas when needed.
  • Assists team members as needs arise, and readily assists in all activities of the unit.
  • Reports abuse or suspected abuse.
  • Demonstrates flexibility in recognizing and participating in 24-hour staffing if need occurs.
  • Accurately inform the on-call management team member (DON, ADON, NP) of acute changes in residents’ condition, staffing problems, and any other emergency.
  • Coordinates night/evening shift operations of care staff to care for Convent, Rosary, and Independent Sisters. Promotes a holistic and interdisciplinary approach to care.
  • Ensures that a robust shift change meeting between night shift team members with evening and day shift team members occur on all floors by rotating participation on each floor / shift.
  • Promotes interdisciplinary communications between all shifts. Assures changes in sisters’ conditions are reported to the appropriate person(s).
  • Gives oral and written reports of pertinent information to oncoming team members when needed.
  • Communicates accurately in oral or written reports.
  • Actively participates in the growth and development of staff through in-services, monthly staff meetings and ongoing supervision.
  • Serves as a scheduler while on call and assures there is proper staffing and reaching out to agency if needed. Working collaboratively with the DON to assure call offs are filled.
  • Recognizes and rewards positive staff behavior. Addresses inappropriate behaviors immediately. Documents these interactions and shares with the staff member.
  • Provides corrective action and completes documentation. Responsible for completing performance evaluations.
  • Reports to the Nurse Coordinators to ensure care journals and care plans are clinically appropriate and include Sister’s preferences and unique needs during night shift.

Personal Care of Sisters

  • Serves as the “expert” or resource to the care partners in problem-solving while promoting independent thinking. Serves as the final decision-maker during times of disagreement. Works closely with the night nurses.
  • Assists in assuring night/evening nurses and care partners receive all information/communications.
  • Assists Care partners with personal care and assuring the welfare and safety of each sister.
  • Assuring that there are no elopement risks and all sisters are accounted for.
  • Reports any issues or concerns to the Nurse Coordinator on that unit.
  • Actively participates in monthly nurses’ meetings/ monthly care partner meetings and shares information with night/evening staff.
  • Assists in assuring a quality orientation is provided to all night/evening staff.
  • Makes nightly rounds on each sister and completes rounding sheets.
  • Completes performance appraisals for the night/evening care partners.
  • Works collaboratively with the Nurse Coordinators at night or evening shift care partners evaluations.
  • Consistently demonstrates professional attitudes and manner including confidentiality.
  • Will be responsible for meeting and staying with the Sister until shift change or a Superior relieves you at the hospital the sister at the hospital, following up with Medical On-Call, and working closely with Superiors to assure that care is provided to the sister who is sent out to the hospital.
  • Participates in nursing diagnosis and therapeutic assessments of Sisters’ conditions by accurately observing, reporting and recording to proper source.
  • A through understand of, medication information, explaining diagnoses, appropriate care and treatment for specific diagnoses, to hospital staff etc.

Other Duties

  • As the whole house supervisor, you will be responsible for assisting and supporting staff and Sisters on floors 2 through 5 of St. Mary’s Convent, Rosary, and Augusta wherever appropriate.
  • Attends at least 80% of staff meetings. Participates regularly in required PDL training and in-service training.
  • Knowledgeable regarding fire and disaster plans.
  • Maintains attendance and observes working hours, starting time, lunch period, breaks, quitting time, etc.
  • Complies with work rules, regulations, and instructions, follows established systems and procedures, completes assignments with minimal supervision.
  • Other duties as assigned.

MINIMUM QUALIFICATIONS

Education

Bachelor’s degree encouraged. Must hold and maintain a license as a Registered Nurse in the state of Indiana.

Experience

Two years of supervisory/management experience preferred, 2 years’ experience working with older adults. Must maintain a valid driver’s license.

Supervision

Responsible for supervision of Care Partners and Staff Nurses, participating in interdisciplinary communication and decision making; planning for staffing and performance management.

Responsibility

Provided appropriate authority and accountability to achieve the scope of their tasks.

Problem-Solving

Will require critical thinking skills. Problems require analytical, evaluative, and constructive thought. May entail consultation with peer group, management, or Religious Superiors.

Physical Demands

While performing the duties of this job, the employee must be able to speak clearly and hear. This position is very active and requires standing, walking, bending, kneeling,

crouching, reaching, pushing, pulling, lifting, stooping, and grasping all day. The employee must frequently lift and/or move items over 20 pounds and occasionally exert up to 50 pounds.

Hazards and Fatigue

Exposure to hazards and fatigue are present intermittently. Exposed to bodily fluids and infectious wastes.
